Thermometer

I came across the attached ad while taking the metro/subway/mtr in Hong Kong. It is a set of ads about a pharmaceutical company and the improvments they have made in human lives. However, this one in particular, the context is just a little strange for my taste. If you cannot read clearly what is said on the ad, or cannot read Chinese, below is a literal translation starting from the text on the left to the right:

"Before, little kids are discomfortable when testing their temperature through their rectum. Now, with ear thermometer, testing temperature is easy and safe."

Ok, the text itself is fine, but accompanied by the picture, I mean, what does the picture suggest? Why couldn't they have a picture of ear thermometer, but instead, must use this one? What are they thinking?
